RESUMO

The present study aimed to review the literature on the main complications of antineoplastic therapies and the degree of knowledge of dental surgeons about these complications. A bibliographic search was conducted in the main health databases PUBMED (www.pubmed.gov) and Scholar Google (www.scholar.google.com.br), in which studies published from 1987 to 2023 were collected. Laboratory studies, case reports, systematic and literature reviews, which were developed in living individuals, about the main neoplastic genes and their relationship with the cells of individuals affected by neoplasms in the head and neck region, and studies on the care with this group of patients, were included. Therefore, articles that did not deal with neoplasm and the main complications of antineoplastic therapies were excluded. Neoplasm is a clonal disorder, caused by mutations, resulting from changes in the genetic structure of cells. Each healthy cell has instructions on how to grow and divi de. In the presence of any error in these instructions (mutation), it can result in a diseased cell that, when proliferating, may cause a tumor. Countless knowledge has been accumulated over the years on the main characteristics of neoplasms, whether they are cancer cell biology, carcinogenesis mechanism, neoplasms of the maxillofacial system and sequels of antineoplastic treatments. In this context, methods have been developed that offer a better quality of life for patients diagnosed with this pathology, as well as preventive vaccine models that may, in the not too distant future, contribute to this goal to be successfully achieved.


El presente estudio tuvo como objetivo revisar la literatura sobre las principales complicaciones de las terapias antineoplásicas y el grado de conocimiento de los odontólogos sobre este abordaje. Se realizó una búsqueda bibliográfica en las principales bases de datos de salud PUBMED (www.pubmed.gov) y Scholar Google (www.scholar.google.com.br), en la que se recopilaron estudios publicados entre 1987 y 2023. Fueron incluidos estudios de laboratorio, relatos de casos, revisiones de la literatura y revisiones sistemáticas, desarrolladas en individuos vivos, que incluyeran los principales genes neoplásicos y su relación con las células de individuos afectados por neoplasias en la cabeza y el cuello. También, se tuvieron en cuenta estudios relacionados con la atención a este grupo de pacientes. La neoplasia es un trastorno clonal, causado por mutaciones, como resultado de cambios en la estructura genética de las células. Cada célula sana tiene instrucciones sobre cómo crecer y dividirse. En presencia de cualquier error en estas instrucciones (mutación), puede provocar una célula alterada que, al proliferar, puede causar un tumor. Se han acumulado innumerables conocimientos a lo largo de los años sobre las principales características de las neoplasias, ya sea sobre biología de células cancerosas, el mecanismo de la carcinogénesis, la neoplasias del sistema maxilofacial y las diferentes secuelas de tratamientos antineoplásicos. En este contexto, se han desarrollado métodos que ofrecen una mejor calidad de vida para los pacientes diagnosticados con esta patología, así como modelos de vacunas preventivas que, en un futuro no muy lejano, pueden contribuir a alcanzar este objetivo con éxito.

The present study proposed to describe, through a literature review, the use of new therapeutic management which allows for offering a better quality of life to individuals affected by these pathologies. A bibliographic search was conducted in the main health databases PUBMED (www.pubmed.gov) and Scholar Google (www.scholar.google.com.br), in which studies published from 1987 to 2023 were collected. In the first stage, the list of retrieved articles was examined by reading the titles and abstracts. In the second stage, the studies were selected by reading the full contents. Two authors (JDMM and DAQ) performed stages 1 and 2. Experimental, clinical, case-control, randomized controlled, and laboratory cohort studies, case reports, systematic reviews, and literature reviews, which were developed in living individuals, were included. Therefore, articles that did not deal with the subject in question, letters to the editor, opinion articles, duplicated literature in databases, and literature that did not address the variables under study, we re excluded. Contemporary dentistry uses alternative treatments capable of improving the patient's condition since a cure is not always possible. Therefore, the possibility of improving the quality of life becomes an important point to be reached. Evidence-based healthcare has made great advances in recent decades, especially in the areas of orofacial pain, TMD, and occlusion, especially related to orthodontic, prosthetic, and restorative care.


En el presente estudio se propuso describir, a través de una revisión bibliográfica, el uso de nuevos manejos terapéuticos que permitan brindar una mejor calidad de vida a los individuos afectados por estas patologías. Se realizó una búsqueda bibliográfica en las principales bases de datos de salud PUBMED (www.pubmed.gov) y Scholar Google (www.scholar.google.com.br), en las que se recopilaron estudios publicados entre 1987 y 2023. En la primera etapa, se examinó la lista de artículos recuperados mediante la lectura de los títulos y resúmenes. En la segunda etapa, los estudios fueron seleccionados mediante la lectura del contenido completo. Dos autores (JDMM y DAQ) realizaron las etapas 1 y 2. Se incluyeron estudios de cohortes experimentales, clínicos, de casos y controles, controlados aleatorios y de laboratorio, informes de casos, revisiones sistemáticas y revisiones de la literatura, que se desarrollaron en individuos vivos. Por lo tanto, se excluyeron artículos que no trataran el tema en cuestión, cartas al editor, artículos de opinión, literatura duplicada en bases de datos y literatura que no abordara las variables en estudio. La odontología contemporánea utiliza tratamientos alternativos capaces de mejorar el estado del paciente, ya que no siempre es posible la curación. Por lo tanto, la posibilidad de mejorar la calidad de vida se convierte en un objetivo importante. La atención médica basada en la evidencia ha logrado grandes avances en las últimas décadas, especialmente en las áreas de dolor orofacial, TMD y oclusión, especialmente en relación con la atención de ortodoncia, prótesis y restauración.

OBJECTIVE: To delineate and discuss the literature about TMD in the context of dental implantology. METHODS: The PRISMA-ScR and Joanna Briggs guidelines for scoping reviews were used as methodological parameters. In vivo studies published in the English language showing interplay between dental implant therapy and TMD were included. A systematic screen strategy was applied in two platforms to obtain a broad range of relevant literature published in English. RESULTS: The literature review indicated that prolonged surgical procedure may be a risk for TMD. Conversely, implant-supported prostheses were found to contribute to the mitigation of some TMD aspects. Empirical principles for the best practice in implant dentistry concerning the TMD were discussed in the present study. CONCLUSION: The included clinical studies suggest that factors associated with the implant placement, such as long duration of surgical procedures, may represent risk factors for TMD. They also indicate long-term benefits of implant-supported restorations.

This study aimed to evaluate the use of bioengineering tools, finite element analysis, strain gauge analysis, photoelastic analysis, and digital image correlation, in computational studies with greater validity and reproducibility. A bibliographic search was performed in the main health databases PUBMED and Scholar Google, in which different studies, among them, laboratory studies, case reports, systematic reviews, and literature reviews, which were developed in living individuals, were included. Therefore, articles that did not deal with the use of finite element analysis, strain gauge analysis, photoelastic analysis, and digital image correlation were excluded, as well as their use in computational studies with greater validity and reproducibility. According to the methodological analysis, it is observed that the average publication of articles in the Pubmed database was 2.03 and with a standard deviation of 1.89. While in Google Scholar, the average was 0.78 and the standard deviation was 0.90. Thus, it is possible to verify that there was a significant variation in the number of articles in the two databases. Modern dentistry finds in finite element analysis, strain gauge, photoelastic and digital image correlation a way to analyze the biomechanical behavior in dental materials to obtain results that assist to obtain rehabilitations with favorable prognosis and patient satisfaction.

RESUMO

O presente trabalho ilustra e discute essa doença que representa o tumor odontogênico mais comum da prática clínica, o Odontoma. Para tal, apresenta um caso clínico com imagens clínicas e radiográficas esclarecedoras dos aspectos dessa doença, bem como discussão a partir de revisão narrativa de literatura direcionada para a carac-terização da doença. Relata-se o caso de um odontoma composto-complexo, uma forma incomum de odontoma, mas que mantém os demais aspectos usuais, sendo uma mulher de 21 anos com lesão radiopaca em maxila de-tectada devido à não erupção do 23. A remoção cirúrgica seguiu a partir de acesso anterior na maxila e o exame histopatológico mostrou os aspectos típicos de um Odontoma composto e complexo juntos. Independentemente da variante, nota-se que o Odontoma é quase sempre indolente, porém requer cuidados de remoção precoce devido à possibilidade de crescimento, de desenvolvimento de cistos e comprometimento de estruturas adjacentes devido a sua localização e possibilidade de crescimento (AU)


The present work illustrates and discusses this disease that represents the most common odontogenic tumor in clinical practice, the Odontoma. However, an unusual variation of the same is included here in the literature, char-acterized by the exams as a form of compound-complex odontoma additionally causing dentigerous cyst formation and dental impaction. The unusual case occurred in a 21-year-old woman, complaining of missing the tooth 23. The details of the exams allowed the visualization of compound and complex areas in the lesion, cystic formation and dental impaction. The histopathological findings confirmed the diagnosis of the Odontoma, and the interpreta-tion with the set of other exams led to the final diagnosis of Compound-complex odontoma with dentigerous cyst. The present case highlights the need for early removal of Odontoma due to the possibility of developing cysts, compromising adjacent structures, in addition to its growth potential already reported in the literature (AU)

ABSTRACT: The present study aims to describe through a literature review, the main types of noble and non-noble alloys in dentistry looking to identify the adhesion mechanisms, compositions and mechanical properties, and its applicability as a rehabilitation resource nowadays. A bibliographic search was conducted in the main health databases PUBMED (www.pubmed.gov) and Scholar Google (www.scholar.google.com.br), in which studies published from 1971 to 2021 were collected. Laboratory studies, case reports, systematic and literature reviews, which were developed in living individuals. Articles that did not deal with metal alloys and its use in dentistry were excluded. Through the review, it was possible to ver ify that all works presented the metal alloys and their main properties, indicating that they are divided into three main types: hi gh noble alloys, noble alloys and base metal alloys differing in their levels of constituent noble metals. Several alloys and meta ls are available for the dental market each presenting advantages and disadvantages, mainly based on its specific composition.Continuous research and development are resulting in the production of new technologies and products, giving dental surgeons even more options in the design and manufacture of restorations using metal alloys and understanding that these resources will still be viable alternatives in oral rehabilitations. However, further studies on metal alloys are needed to better understand this subject.


RESUMEN: El presente trabajo tuvo como objetivo describir a través de una revisión de la literatura, los principales tipos de aleaciones nobles y no nobles utilizados en odontología buscando identificar los mecanismos de adhesión, composiciones y propiedades mecánicas, así como reflejar su aplicabilidad como recurso rehabilitador en la actualidad. Realizamos una búsqueda bibliográfica em las principales bases de datos de salud PUBMED (www.pubmed.gov) y Scholar Google (www.scholar.google.com.br), en la que se recopilaron estudios publicados desde 1971 hasta 2021. Estudios de laboratorio, informes de casos, revisiones sistemáticas y bibliográficas, que se desarrollaron en individuos vivos. Sin embargo, se excluyeron los artículos que no trataban sobre aleaciones metálicas y su uso en odontología. Se pudo observar que todos los trabajos presentaban las aleaciones metálicas y sus principales propiedades indicando que se estas dividen en tres tipos principales: aleaciones altamente nobles, aleaciones nobles y aleaciones de metales base que difierenen sus niveles de metales nobles constituyentes. Hay varias aleaciones y metales disponibles para el mercado dental, cada uno presenta ventajas y desventajas, principalmente en función de su composición específica. La investigación y el desarrollo continuo están dando como resultado la producción de nuevas tecnologías y productos, brindando a los cirujanos dentistas aún más opciones en el diseño y fabricación de las restauraciones, utilizando aleaciones metálicas y, permite concluir que estos recursos seguirán siendo alternativas viables en los tratamientos de rehabilitación. Sin embargo, se necesitan más estudios sobre el tema abordado en el trabajo, para una comprensión más profunda del tema.

ABSTRACT: The aim of this study was to identify the incidence of fractures in single crowns placed in individuals with chronic tooth wear. The sample selection was performed based on the evaluation of 218 patient records rehabilitated with single crowns placed by the same professional from a private clinic. After exclusion criteria, 43 individuals (18 males and 25 females) were included and a total of 112 single crowns distributed in both jaws. Three ceramic systems were evaluated: 34 feldspathic all-ceramic, 38 metal-ceramic with a core in gold electropositive alloy, and 40 In-Ceram Alumina. The intraoral photograph analysis adopted a diagnosis method of tooth wear according to the morphological variations of the teeth. Of the 112 crowns evaluated, 47 were placed in male and 65 in female patients, aged between 21 to 74 years (mean 27.5 years old). The variables gender (p = 0.83), presence of chronic tooth wear (p = 0.91), ceramic system (p = 0.24) and crown location (p = 0.86) did not present a statistically difference. However, the variables pulp vitality (p = 0.04) and survi val time (p = 0.01) presented a statistically significant difference. The evaluated ceramic systems can be considered as alternativ es of restorative material, even in individuals with clinical characteristics suggestive of chronic tooth wear.


RESUMEN: El objetivo de este estudio fue identificar la incidencia de fracturas en coronas individuales colocadas en individuos con desgaste dental crónico. La selección de la muestra se realizó en base a la evaluación de 218 registros de pacientes rehabilitados con coronas individuales colocadas entre 1999 y 2009 por el mismo profesional de una clínica privada. Después de analizar los criterios de exclusión, se incluyeron 43 individuos (18 hombres y 25 mujeres) y un total de 112 coronas individuales distribuidas en ambas mandíbulas. Este estudio evaluó coronas hechas de tres sistemas cerámicos: 34 de cerámica totalmente feldespática, 38 de cerámica de metal con un núcleo de aleación electropositiva de oro y 40 de alúmina enceram. El análisis de la fotografía intraoral adoptó un método de diagnóstico del desgaste dental de acuerdo con las variaciones morfológicas de los dientes. De las 112 coronas evaluadas, 47 se colocaron en hombres y 65 en mujeres, con edades comprendidas entre 21 y 74 años (media de 27,5 años). Con respecto a las coronas individuales, las variables género (p = 0,83), presencia de desgaste dental crónico (p = 0,91), sistema cerámico (p = 0,24) y ubicación de la corona (p = 0,86) no presentaron una diferencia estadísticamente significativa. Sin embargo, las variables vitalidad pulpar (p = 0.04) y tiempo de supervivencia (p = 0.01) presentaron una diferencia estadísticamente significativa. Los sistemas cerámicos evaluados pueden considerarse como alternativas de material restaurador, incluso en individuos con características clínicas que sugieren desgaste dental crónico.

ABSTRACT: The present study aims to describe, through a case report, the functioning of bone grafts, where they are obtained including bone banks and the different dental areas in which it is used. A 50-year.old female patient HMR, sought the postgraduate college of dentistry CECAPE for oral rehabilitation. The initial clinical examination revealed the absence of several dental elements in both the maxilla and mandi ble, bone resorption in the entire maxillary extension where there was an absence of elements in the anterior region and the presence of a torus in the palatal region. Initial tomographic examinations of the maxilla and mandible were performed, looking for the best conduct regarding the diagnosis and planning of the case. After the clinical and radiographic examinations was performed, and the diagnosis was reached, it was determined as a treatment for oral rehabilitation of the maxilla, bone grafting and an Implant-supported Removable Partial Prosthesis and for the mandible a Dento-muco-supported Removable Partial Prosthesis. Dental treatments and oral rehabilitation from the use of bone tissues and bone grafting have become commonly used. Procedures that would normally lead to failure started to increase the percentage of successes due to the use of these biomaterials. The use of bone grafts in dentistry has been shown to be a very effective and useful resource, considering its great applicability in bone reconstructions in different areas of dentistry.


RESUMEN: El presente estudio tiene como objetivo describir a través de un informe de caso, el funcionamiento de los injertos óseos, donde se obtienen, incluidos los bancos de huesos y las diferentes áreas dentales en las que se emplea. Una paciente HMR, de 50 años de edad, asistió el centro universitario de posgrado CECAPE para rehabilitación oral. El examen clínico inicial reveló la ausencia de varios dientes tanto en el maxilar como en la mandíbula, la resorción ósea en toda la extensión maxilar donde había ausencia de dientes en la región anterior y la presencia de un torus en la región palatina. Se realizaron exámenes tomográficos iniciales del maxilar y mandíbula, buscando la mejor conducta con respecto al diagnóstico y la planificación del caso. Después de los exámenes clínicos y radiográficos realizados y el diagnóstico listo, se determinó como un tratamiento para la rehabilitación oral del maxilar, el injerto óseo y una prótesis removible implantosoportada y para la mandíbula una prótesis parcial removible dento-muco-soportada. Los tratamientos dentales y la rehabilitación oral a partir del uso de tejidos óseos y el injerto óseo se han utilizado comúnmente. Los procedimientos que normalmente conducirían al fracaso comenzaron a aumentar el porcentaje de éxitos debido al uso de estos biomateriales. Se ha demostrado que el uso de injertos óseos en odontología es un recurso muy eficaz y útil, considerando su gran aplicabilidad en las reconstrucciones óseas en diferentes áreas de la odontología.

To evaluate clinically the results of two surgical techniques used for root coverage, the coronally advanced flap in '' L '' isolated (CAF) or associated with subepithelial connective tissue graft (SCTG). The surgical procedures were performed in seventeen individuals, ten individuals in the control group (coronally advanced flap in "L") and seven individuals in the test group (coronally advanced flap in "L" associated with SCTG), who presented Miller class I and II gingival recession. The depth gingival recession (GR) index was evaluated in the following periods: immediate preoperative and 180 postoperative days. The control group had a success rate of 85 +/- 18 % and the test group had a success rate of 95 +/- 4 %. The variable success rates (p=0.36) did not present a statistically significant difference. The isolated CAF or its association with SCTG showed favorable outcomes in the treatments of Miller's class I and II gingival recessions.


El objetivo fue evaluar clínicamente los resultados de dos técnicas quirúrgicas utilizadas para la cobertura radicular, el colgajo coronalmente avanzado en "L" aislado (CAF) o asociado con injerto de tejido conectivo subepitelial (SCTG). Los procedimientos quirúrgicos se realizaron en diecisiete individuos, diez individuos en el grupo de control (colgajo coronario avanzado en "L") y siete individuos en el grupo de prueba (colgajo coronalmente avanzado en "L" asociado con SCTG), que presentaron la clase I de Miller y II recesión gingival. El índice de recesión gingival profunda (RG) se evaluó en los siguientes períodos: preoperatorio inmediato y 180 días postoperatorios. El grupo de control tuvo una tasa de éxito de 85 +/- 18 % y el grupo de prueba tuvo una tasa de éxito de 95 +/- 4 %. Las tasas de éxito variables (p = 0,36) no presentaron una diferencia estadísticamente significativa. La CAF aislada o su asociación con SCTG mostraron resultados favorables en los tratamientos de las recesiones gingivales de clase I y II de Miller.

AIMS: The aim of the study is to evaluate the profile of peri-implant tissues in periodontally compromised patients. SETTINGS AND DESIGN: In vivo - cross sectional study design. MATERIALS AND METHODS: Fifty-eight implants were evaluated, clinically and radiographically, installed in seven individuals treated by the same team of professionals, during the years 1997 and 2005 in a private dental clinic in Vitória, ES, Brazil; that time of data collection, all implants were at least 10 years of functional loading. The variables related to the dental implants evaluated were: visible Plaque Index, Gingival Bleeding Index (GBI), probing pocket depth, bleeding on probing, and bone level, to relate them to the classification of dental implants. STATISTICAL ANALYSIS USED: The Chi-square and Kruskal-Wallis test were adopted. RESULTS: The total of 58 implants were classified: 11 (18.9%) as healthy and 12 (20.7%) as clinically stable. The other 35 implants (60.4%) had some type of peri-implant inflammation, 20 of them (34.5%) were diagnosed with peri-implant mucositis and 15 (25.9%) with peri-implantitis. Among the variables studied, the results showed statistically significant differences for implant location (P = 0.001) and GBI (P = 0.03). Most of the maxillary implants (85.7%) were classified for some type of peri-implant disease. For the implants which resulted in Score 1 for GBI, most of them (75.0%) were also classified for some type of peri-implant disease. CONCLUSIONS: Dental implants placed in periodontally compromised patients may have high long-term survival rates. However, most implants were classified with some type of peri-implant inflammation.

RESUMEN: El objetivo de este trabajo consistió en evaluar la resistencia al cizallamiento de brackets ortodónticos cementados a una cerámica odontológica a base de disilicato de litio. Fueron confeccionadas 80 muestras rectangulares de cerámica vítrea a base de disilicato de litio. Las muestras fueron divididas inicialmente en 4 grupos de 20 discos según el tratamiento de superficie: ácido fosfórico 37 % por 30 segundos (HP); ácido hidrofluorhidro 10 % por 60 segundos (HF); óxido de aluminio durante 4 segundos (SB); y aspersión con punta diamantada durante 10 segundos (DW). Cada grupo fue dividido en dos subgrupos para la cementación de brackets Edgwise: metálico y cerámico. Los dos nuevos grupos fueron subdivididos en termociclados o no termociclados. Las muestras termocicladas pasaron por 500 ciclos con variaciones de temperatura entre 5 °C a 55 °C. Los valores numéricos obtenidos en la prueba de cizallamiento se sometieron al análisis estadístico descriptivo (media y desviación estándar), habiéndose realizado la prueba de normalidad Shapiro-Wilk y las pruebas no paramétricas de Kruskal-Wallis y Mann-Whitney. Se verificó que no hubo diferencia estadísticamente significativa en relación al tipo de tratamiento de superficie y la realización o no de termociclado, observándose diferencia estadísticamente significativa sólo en el tipo de bracket, siendo la resistencia de unión en el bracket metálico mayor que en el de cerámica. Así, el tipo de tratamiento de superficie y el envejecimiento por termociclaje no fueron decisivos en la resistencia de unión de brackets ortodónticos a la cerámica de disilicato de litio.


ABSTRACT: To evaluate the shear strength of cemented orthodontic brackets to a dental ceramic based on lithium disilicate. Rectangular samples 80 based glassceramic lithium disilicate were prepared. The samples were initially divided into 4 groups of 20 discs according to surface treatment: 37 % phosphoric acid for 30 seconds (HP); 10 % hydrofluoric acid for 60 seconds (HF); blasting with aluminum oxide for 4 seconds (SB); and diamond-tipped for 10 seconds (DW). Each group was divided into two subgroups for the Edgwise bracket cementation: metallic and ceramic. The two new groups were subdivided into thermocyclers or nonthermocyclics. The thermocycled samples passed 500 cycles with temperature variations between 5 ° C to 55 ° C. The numerical values obtained in the shear test were submitted to descriptive statistical analysis (mean and standard deviation), and the Shapiro-Wilk normality test and nonparametric Kruskal-Wallis and Mann-Whitney tests were performed. It was found that there was no statistically significant difference in the type of surface treatment and conducting or not thermocycling, observing statistically significant difference only on the bracket type, and the bond strength at higher metal bracket than in ceramics. Thus, the type of surface treatment and thermocycling aging were not decisive in the bond strength of orthodontic brackets to the lithium disilicate ceramic.

Objetivo: realizar uma revisão de literatura sobre a ocorrência de alterações significativas na pressão arte¬rial sistêmica de pacientes submetidos a procedimentos cirúrgicos odontológicos. Materiais e método: esta revisão de literatura foi conduzida por meio das seguintes bases de dados: PubMed, SciELO e Google Scholar. As palavras-chave para a busca textual foram: cirurgia oral (oral surgery), hipertensão (hypertension), ansiedade (anxiety), odontologia (dentistry). Os critérios de inclusão foram: literatura sobre a temática em estudo, literatura dos últimos anos, literatura publicada em inglês ou português, estudos laboratoriais e clínicos e revisão sistemática. Os critérios de exclusão foram: revisão de literatura, carta ao editor, artigo de opinião, literatura duplicada em bases de dados e literatura que não abordasse as variáveis em estudo. Revisão de literatura: durante a prática clínica, o cirurgião-dentista lida rotineiramente com pacientes sistemicamente comprometidos. Entre eles, encontram-se os pacientes hipertensos, que necessitam de uma atenção diferenciada, devido a diversos fatores externos que podem influenciar sua condição sistêmica. Considerações finais: existe uma variação da pressão arterial durante todo procedimento cirúrgico odontológico, e o momento mais evidente dessa alteração é após a anestesia; essas variações, por sua vez, podem ser atribuídas à ansiedade e ao estresse causados pelo ato cirúrgico; a redução do estresse bem como o controle da ansiedade e do medo frente a um tratamento odontológico são benefícios no atendimento de pacientes hipertensos. Objective: the objective of this study was to describe through a review of the literature on the occurrence of significant changes in the systemic arterial pressure of patients submitted to dental surgical procedures. Materials and method: this literature review was conducted using the databases: PubMed, SciELO and Google Scholar. The keywords for the literature search were: Oral surgery; Hypertension; Anxiety; Dentistry. The inclusion criteria were: papers that covers the subject under study, papers of the last years, papers in English and Portuguese language, laboratory studies, clinical studies and systematic review. The exclusion criteria were: literature review, letter to the editor, opinion article, papers duplicated in databases and literature that did not address the variables under study. Literature review: in the clinical practice, the dental surgeon routinely treats systemically compromised patients. Among them are the hypertensive patients who need a differentiated attention due to several external factors that can influence their systemic health. Final considerations: there are a variation of the arterial pressure throughout the dental surgical procedure, the moment that this change becomes more evident is after the anesthesia; these variations in turn can be attributed to the anxiety and stress caused by the surgical act; the reduction of surgical stress as well as the control of anxiety and fear of dental treatment are benefits in the care of hypertensive patients. Os critérios de inclusão foram: literatura que aborde a temática em estudo, literatura dos últimos anos, idiomas inglês e português, estudos laboratoriais e clínicos e revisão sistemática. Os critérios de exclusão foram: revisão de literatura, carta ao editor, artigo de opinião, literatura duplicada em bases de dados e literatura que não abordasse as variáveis em estudo. Revisão de literatura: a bioética baseia-se em quatro princípios básicos que estabelecem uma metodologia para analisar os casos concretos e os problemas éticos que ocorrem na prática da assistência à saúde, sendo eles: não maleficência, beneficência, respeito à autonomia e justiça. Sendo esses princípios fundamentais para o desenvolvimento da bioética, com isso, abordando uma forma peculiar de definir e manejar os valores envolvidos nas relações dos profissionais de saúde e seus pacientes. Considerações finais: a bioética deve nortear os avanços dentro do respeito ao ser humano e à sua individualidade, sem que ocorram possíveis infrações. Todavia, são necessários maiores estudos para uma maior compreensão sobre o tema e as melhores medidas para cada caso em particular. (AU)


Objective: to review the literature on the means by which dentists should guide their behavior, becoming aware of the requirements concerning their profession. Materials and method: this literature review was performed by the main health databases of PubMed, Sci- ELO, and Google Scholar. The keywords for the text search were ethics, professional; education, dental; dentistry. The inclusion criteria were the literature addressing the subject studied, the recent literature, English and Portuguese languages, laboratory and clinical studies, and systematic reviews. The exclusion criteria were literature reviews, letters to the editor, opinion articles, the literature duplicated in databases, and the literature that did not address the variables studied. Literature review: bioethics is based on four basic principles that establish a methodology for analyzing the concrete cases and ethical problems occurring in the health care practice, as follows: non-maleficence, beneficence, respect for autonomy, and justice. These are fundamental principles for the development of bioethics, hence addressing a particular way of defining and managing the values involved in the relationships between health professionals and their patients. Final considerations: bioethics should guide the advances regarding human beings and their individuality, without the occurrence of potential infractions. However, further studies are required for a better understanding of the subject and for the best measures for each particular case. (AU)

Objetivo: descrever uma revisão da literatura que apresente as principais consequências que podem ocorrer quando o dente avulsionado é reimplantado de forma tardia, proporcionando um prognóstico desfavorável. Revisão de literatura: a avulsão dental é uma lesão traumática que se caracteriza pelo completo deslocamento do dente de seu alvéolo, acarretando danos tanto às estruturas de suporte do elemento dental quanto às estruturas pulpares. A permanência extraoral do elemento dental avulsionado por períodos longos ou em meios de armazenamento inadequados pode provocar danos adicionais. Considerações finais: as lesões de inserções são as principais consequências da pós-avulsão dentária, isso devido a uma ruptura do ligamento periodontal, com uma secagem excessiva antes do reimplante, danificando as células do ligamento periodontal, o que, por sua vez, provoca uma resposta inflamatória exacerbada em uma ampla área da superfície radicular. Objective: to describe a literature review, which presents the main consequences of late reimplantation of the avulsed tooth, providing an unfavorable prognosis. Literature review: tooth avulsion is a traumatic lesion characterized by the complete displacement of the tooth from the socket, causing damage to the support structures of the dental element as well as to pulp structures. The long extraoral time of the avulsed tooth or the time in inadequate storage may cause additional trauma. Final considerations: insertion lesions are the main consequences of tooth post-avulsion due to a rupture in the periodontal ligament, with an excessive drying prior to reimplantation, which damages the periodontal ligament cells and causes an exacerbated inflammatory response in a large root surface area. RESUMEN: El objetivo de esta investigación fue analizar la etiología de las pérdidas dentales en pacientes rehabilitados con prótesis sobre implantes. Se realizó un estudio transversal con informaciones de 93 pacientes de una clínica privada que recibieron tratamiento con implantes en el período de 2000 a 2009. Los datos fueron recolectados a través de las fichas clínicas, documentaciones radiográficas y fotografías de los casos clínicos. Se levantaron datos sobre género, edad, nivel de escolaridad, hábito de fumar, tipo de diente extraído y razón de la extracción. Los resultados mostraron que la razón periodontal fue el principal motivo de las pérdidas dentales, representando el 24,7 %, seguida por las fracturas radiculares con pines metálicos (23,7 %) y caries secundarias (15,1 %). En los pacientes menores de 40 años, los traumatismos dentales y las fracturas radiculares con pino fueron responsables de la mayoría de las pérdidas. Las razones de las pérdidas dentales mostraron significancia estadística con la variable edad (p<0,05). El género, el hábito de fumar y el grado de escolaridad no mostraron asociación significativa (p>0,05) con las razones de las pérdidas dentales.


ABSTRACT: The aim of this study was to analyze the etiology of tooth loss in patients rehabilitated with dental implants. Was conducted a cross-sectional study with data from 93 patients from a private clinic who were treated with implants from the year 2000 until 2009. Data were collected through medical records, radiographic documentation and photographs of clinical cases. Data on sex, age, education level, smoking habits, teeth loss localization and extraction ratio were analyzed. The results showed that periodontal disease was the main reason for tooth loss, accounting for 24.7 %, followed by root fractures with posts (23.7 %) and secondary caries (15.1 %). In patients below 40 years, dental trauma and root fractures were responsible for most of the losses. The reasons for tooth loss showed statistical significance with the age (p<0.05). Sex, smoking habits and education showed no significant association (p>0.05) with the reasons of the tooth loss.

A atrofia óssea devido a perdas dentárias ocasiona diminuição da altura e espessura do reordo, tornando a mandíbula susceptível a fraturas. Pode-se considerar atrófica, quando a altura óssea não ultrapassa 20 mm. Essas fraturas acometem principalmente a população idosa, porque além da atrofia óssea subsequente ao edentulismo, a redução da vascularização contribui para o enfraquecimento da mandíbula nos pacientes desse grupo etário. O tratamento dessas fraturas permanece um desafio cirúrgico e não há consenso quanto à melhor técnica de tratamento. Isso devido a fatores inerentes a essa patologia, a relativa raridade da mesma, o que gera inexperiência dos cirurgiões e a escassez de informação sobre o tema na literatura cirúrgica. O objetivo deste trabalho consiste em descrever um caso de tratamento de fratura em mandíbula atrófica, no qual foi realizada a correção cirúrgica da fratura através da técnica de redução aberta com acesso intrabucal que se estendeu da região retromolar direita à mesma região contralateral e de segmentos ósseos fixados com uma placa de reconstrução do sistema 2.0 mm, tipo locking e parafusos bicorticais (AU).


Bone atrophy due to tooth loss causes a decrease in bone height and thickness, making jaw susceptible to fractures. It can be considered atrophic when the bone height does not exceed 20 mm. These fractures mainly occlude the elderly population, because in addition to bone atrophy subsequent to edentulism, the reduction of vascularization contributed to mandible weakening in the patients of this age group. The treatment of these fractures remains a surgical challenge and there is no consensus in relation to the best treatment technique. This is due to factors inherent to this pathology, its relative rarity, which generates surgeon's inexperience, and scarcity of information on the subject in surgical literature. The aim of this work is to describe a clinic case of fracture treatment in an atrophic mandible, in which surgical fracture correction was performed through the technique of open reduction and intrabuccal access that extended from the right retromolar region to the same contralateral region and the bone segments fixed with a 2.0mm system rebuilding plate, locking type and bicortical screws (AU).

OBJECTIVES: To evaluate the influence of the type of osteotomy in the inferior aspect of the mandible on the mechanical performance. MATERIALS AND METHODS: The study was performed on 20 polyurethane hemimandibles. A sagittal split ramus osteotomy (SSRO) was designed in 10 hemimandibles (group 1) with a vertical osteotomy in the buccal side (second molar level) and final osteotomy was performed horizontally on the lingual aspect, while the mandible body osteotomy was finalized as a straight osteotomy in the basilar area, perpendicular to the body. For group 2, the same osteotomy technique was used, but an oblique osteotomy was done in the basilar aspect of the mandibular body, forming continuity with the sagittal cut in the basilar area. Using a surgical guide, osteosynthesis was performed with bicortical screws using an inverted L scheme. In both groups vertical compression tests were performed with a linear load of 1 mm/min on the central fossa of the first molar and tests were done with models made from photoelastic resin. Data were analyzed using Student's t-test, establishing a statistical significance when P <0.05. RESULTS: A statistical difference was not observed in the maximum displacements obtained in the two osteotomies (P <0.05). In the extensiometric analysis, statistically significant differences were identified only in the middle screw of the fixation. The photoelastic resin models showed force dissipation towards the inferior aspect of the mandible in both SSRO models. CONCLUSION: We found that osteotomy of the inferior aspect did not influence the mechanical performance for osteosynthesis with an inverted L system.

O objetivo do presente trabalho foi relatar um caso clínico de dilaceração e inversão dentária após trauma dentário. Nos primeiros estágios de desenvolvimento, o germe do incisivo superior permanente se encontra situado palatalmente e superiormente ao ápice do incisivo central decíduo. Gradualmente, o sucessor permanente muda em direção labial. Nesse momento a coroa do dente permanente avizinha da raiz do sucessor, iniciando o processo de reabsorção do dente decíduo. Por razões desta estreita relação entre o germe do dente permanente e o ápice do incisivo decíduo, acredita-se que um trauma agudo ao antecessor decíduo pode causar a dilaceração do eixo longitudinal do sucessor permanente. Clinicamente, a dilaceração pode ser revelada por meio de palpação alta no sulco labial e/ou no palato duro, enquanto que o exame radiográfico é característico e decisivo para o diagnóstico. A abordagem terapêutica de incisivo central superior tem de ser cuidadosamente planejada e precisa da cooperação de várias especialidades para alcançar o objetivo. A extração do incisivo dilacerado é uma prática comum, mas compromete o paciente com um extenso tratamento reabilitador no futuro...


The objective of this study was to present a case report of a dilacerated and inverted tooth after dental trauma. In the early stages of development, the germ of the permanent incisor is situated palatally and above the apex of the deciduous central incisor. Gradually the permanent successor changes in labial direction, at that time the permanent tooth crown approaches to the successor tooth root, starting the process of resorption in deciduous teeth. Due to this proximal relation between the germ of the permanent tooth and the apex of the deciduous incisors, it is believed that an acute trauma to the deciduous predecessor may cause dilaceration of the longitudinal axis of the permanent successor. Clinically, the dilaceration can be revealed by palpation in the labial sulcus and/or the hard palate, and radiographic examination is determinant for a final diagnosis. The treatment of the dilacerated central incisor has to be carefully planned and requires the cooperation of various specialties. The dilacerated incisor extraction is a common practice that may compromise patients’ life requiring an extensive rehabilitation treatment in the future...

The aim of this study was to establish the influence of the design of the sagittal split ramus osteotomy (SSRO) on stress distribution on the osteosynthesis in a photoelastic resin model. Two polyurethane hemimandibles were used to perform the osteotomies, tilted in the lateral sector of the first/second molar (group I) and the other descending downwards and laterally from the first molar (group II), with no higher angle. Six replicas of each were made in photoelastic resin and stabilized with a plate and 5 mm monocortical screws in a standardized way. Stabilization was done in the SSRO without advancement, with 3 mm advancement and with 7 mm advancement. Compressive loads were applied at the level of the lower first molar in an Instron machine (model 4411) with a speed of 1 mm/min until reaching 3 mm of displacement, at which point the data was recorded with a camera to identify the stress distribution bands. The results showed stress distribution in different places: for group I it was observed mainly in the screws of the proximal segment, being more intense closer to the osteotomy; in group II I it was observed mainly in the screws of the proximal segment furthest from the osteotomy, also being distributed towards the upper area of the plate. It may be concluded that under standard osteosynthesis conditions, modifications to the SSRO produce changes in the location and distribution of stress.

El objetivo de esta investigación es identificar las tensiones presentes en tornillos de osteosíntesis utilizados en la fijación de la osteotomía sagital de rama mandibular (OSRM). Se utilizaron dos modelos de OSRM realizados con resina fotoelástica donde el grupo 1 presentaba una osteotomía basilar recta mientras que el grupo 2 presentaba una osteotomía basilar angulada; en ambos se utilizo el sistema de L invertido para la osteosíntesis con tornillos 2.0 de 13 mm de longitud. Posteriormente el sistema fue instalado en un pilar sobre la maquina Instrom, donde se aplico una carga vertical de 1 mm/minuto hasta conseguir el desplazamiento de 3 mm; el punto de carga estuvo en la fosa central del primer molar. La distribución de franjas mostro mayor tensión en el tornillo mas próximo de la osteotomía en ambos grupos; en el grupo 2 se produjo una diseminación de fuerzas levemente hacia la basilar y en el grupo 1 se mantuvo la diseminación a través de la línea oblicua. Se puede concluir que ambas osteotomías mandibulares son eficientes en la distribución de cargas y que no se observan diferencias cualitativas entre tornillos en términos de cantidad de diseminación de energía


The aim of this research was to analyzed the tension in osteosynthesis screw used for fixation of sagittal split ramus osteotomy (SSRS). Was used two model of SSRS made in photoelastic resin; in one of them was include a basilar osteotomy and the another one only SSRS without basilar osteotomy; for both system was used inverted L osteosynthesis with 13 mm screw of 2.0 system. Posteriorly, the hemimandible was installed in Instrom machine with a vertical load of 1 mm/min until the displacement of 3 mm; the load point was on first molar. The fringe distribution show more tension in the screw close to osteotomy in both groups; when SSRS was united to basilar osteotomy was produced a inferior distribution of force and in the SSRS without basilar osteotomy group was maintained a distribution into the oblique line. It's was conclude that both mandibular osteotomies was efficient in load distribution and was not observed important differences between screw related to distribution of load
